Introduction

The healthcare infrastructure is changing rapidly due to changes in demographics and workforce availability. Treatments that previously were only given in the in-patient setting is now being offered in the patient’s own home. This shift from in-patient care to home care creates new possibilities and challenges for hospital pharmacy services. This change can be a daunting challenge in terms of adapting the provided services, but by using each other as inspiration and discussion partners we can further develop our services to benefit our patients.
